Tendinopathy vs. Tendinitis: Why the Distinction Matters
For decades, overuse tendon injuries were called tendinitis — inflammation of the tendon. Treat the inflammation, the thinking went, and the tendon heals. The problem is that research since the 1990s has consistently shown that most chronic tendon pain involves very little actual inflammation. Instead, what happens is a structural breakdown of the tendon’s collagen organization, a process called tendinopathy.
This distinction matters clinically because tendinopathy does not respond to the same treatments as tendinitis. Anti-inflammatory medications, rest, and ice may provide short-term relief but do not address the underlying structural problem. What tendons with chronic pathology need is progressive, controlled loading — the opposite of what many athletes instinctively do (which is rest until the pain goes away and then return to full training).
In archery, shoulder tendinopathy most commonly affects the rotator cuff tendons (particularly the supraspinatus) and the long head of the biceps tendon, which also passes through the shoulder. Both are subjected to repetitive high-load cycles in the archery draw, making them vulnerable when training volume or intensity increases too quickly.
Common Archer Patterns That Lead to Tendinopathy
Shoulder tendinopathy in archery is almost always a story of doing too much too fast. The specific patterns vary but follow recognizable themes.
Arrow count spikes. An archer who shoots 50 arrows a week through winter and then jumps to 200 arrows a week in spring competition season has not given their tendons time to adapt to the fourfold load increase. Muscles adapt within two to three weeks of new training stimulus. Tendons take six to twelve weeks. The gap between muscular readiness and tendon readiness is where overuse injuries live.
Poundage jumps. Adding 10 or 15 pounds of draw weight at the start of a new season — perhaps because you found a great deal on heavier limbs — is one of the fastest routes to shoulder tendinopathy. No rest days. Many committed archers shoot seven days a week when a competition is approaching. Tendons do not adapt during loading — they adapt during recovery. An archer who never gives their shoulder tendons a rest day is simply accumulating damage without allowing the repair cycle to keep pace. Overtraining is not dedication; it is a biology problem.
Technical flaws under fatigue. Late in a long session, form degrades. The draw shoulder elevates, the scapula tips forward, and the supraspinatus gets pinched in the subacromial space with every draw. The first 50 arrows of a session may be mechanically sound; the last 50, shot through fatigue, may be systematically damaging the same tendon with every repetition.
Recognizing the Symptoms of Shoulder Tendinopathy
Shoulder tendinopathy presents as a dull, aching pain localized to the front or side of the shoulder, often described as deep rather than surface-level. It is typically worse at the start of activity, may “warm up” after a few minutes, and then worsens again as fatigue sets in during longer sessions.
One useful self-test: does the pain behave reactively to load? That is, does a heavy shooting session leave the shoulder aching for more than 24 hours? Does the discomfort increase over successive days of shooting without a rest day? These reactive patterns suggest tendinopathic load sensitivity — the tendon is not recovering between sessions.
Unlike an acute injury, tendinopathy creeps in gradually. Many archers ignore the early warning signs — a shoulder that feels “off” for a few days, mild aching that goes away after warming up — until the pain becomes impossible to ignore. By then, the tendon is in a significantly more degraded state and recovery takes proportionally longer.
Load Management: The 10% Rule and Why It Works
The 10% rule is a simple guideline from sports medicine: do not increase total training volume by more than 10% per week. For an archer shooting 100 arrows per week, that means adding no more than 10 arrows the following week, then no more than 11 the week after that. It feels frustratingly slow, particularly for competitive archers who want to shoot high volumes quickly. But tendons operate on their own timeline, and the 10% rule exists because it keeps volume increases within the tendon’s adaptive capacity.
Apply the same principle to draw weight. A 5% increase in poundage per month — held there for a full month before progressing — gives tendons adequate adaptation time. Rushing this progression is the most common mistake archers make when trying to shoot heavier bows.
Also build recovery weeks into your training. Every three to four weeks of progressive loading, schedule a week at 60 to 70% of your peak volume. These cutback weeks allow accumulated tendon stress to resolve before the next loading block begins. Elite archers and their coaches have been using periodized training for decades; the same principles apply at every level.
Isometric Exercises for Tendon Health
Isometric exercises — where the muscle contracts but the joint does not move — have emerged as one of the most effective tools for managing tendinopathic pain and stimulating tendon remodeling. For the shoulder, isometric external rotation is a useful starting exercise.
Stand with your elbow at 90 degrees, forearm parallel to the floor. Press your forearm against a fixed object — a wall, a door frame, or your other hand providing manual resistance — and push outward without actually moving. Hold the contraction for 30 to 45 seconds. Repeat three to five times. Perform this once or twice daily, or before and after shooting sessions. You should feel effort in the posterior shoulder; if you feel pain, reduce the intensity of the contraction.
Isometrics work well early in tendinopathy management because they load the tendon without the repetitive mechanical stress of dynamic exercise. They also have an analgesic effect that can reduce tendon pain for several hours after the exercise — a useful window for shooting practice when you are managing a flare-up.
Return-to-Shooting Protocol
If shoulder tendinopathy has forced you off the range, a graduated return is essential. A reasonable framework: start at 50% of your pre-injury arrow count and 70% of your pre-injury draw weight, with two rest days between sessions for the first two weeks. Monitor the 24-hour response after each session — if pain is below a four out of ten and returns to baseline within 24 hours, you are managing the load appropriately.
Progress arrow count before poundage. When your pre-injury arrow count feels effortless and the shoulder response is consistently minimal, begin adding poundage in small increments (2 to 3 pounds at a time) with at least two to three weeks between increases.
